Hazard Vulnerability Analysis (HVA) Survey - Due 1/31/2024

Hello HCRI Members,

We know many of you are familiar with the Hazard Vulnerability Analysis (HVA). For those of you who are not, the HVA provides the Healthcare Coalition with a list of your most urgent risks, and it gives us direction on what to prioritize in a preparedness capacity. This is a national requirement as well for certain agencies within the Coalition.

Therefore, we would like to gather your agency-specific HVA information. By January 31, 2024, please complete the online HCRI survey located here: https://www.surveymonkey.com/r/STGFP6M. A copy of the HCRI survey is also attached to this notification, and we recommend you use it to collect your answers first. Your answers will reflect how probable each hazard is and how much it will impact you, on a scale of 0-3. All responses must be submitted electronically, via the above SurveyMonkey link.

This HCRI survey has been developed to reflect the Kaiser Permanente HVA Tool: https://www.calhospitalprepare.org/post/updated-hva-tool-kaiser-permanente. It is recommended that your organization first complete the Kaiser-Permanente HVA (above), and then report results from it into this survey. These surveys are reviewed at the agency level, collated by discipline, and used to inform input into the State’s Hazard Identification and Risk Assessment (HIRA) and Threat and Hazard Identification and Risk Assessment (THIRA) that inform the State Hazard Mitigation Plan.
